125. Long-acting Reversible Contraception in Gender Diverse Adolescents and Young Adults: Outcomes from a Multi-site Collaborative

To our knowledge, no studies have specifically sought to understand the use of long-acting reversible contraception (LARC) by gender diverse adolescents and young adults (AYA). We describe characteristics AYA seeking LARC, indications for placement, complications, side effects continuation/removal rates. This retrospective cohort study included age 13-26 years LARC from three U.S. Adolescent Medicine clinics January 2020 March 2022. Baseline demographic information, procedural patient experience continuation rates were assessed. Descriptive statistics characterized population compared outcomes those receiving an intrauterine device (IUD) or implant. Cox proportional-hazard modeling examined time discontinuation device. IRB approval was obtained at each institution. There 63 attempted insertions during period with a mean participant 17.7 years; majority white (67%). No patients had ever been pregnant. Over half (54%) desired both menstrual management purposes, while 30% only 16% only. The 52 mg levonorgestrel IUD most commonly placed (78%, n=28); others 19.5 (n=1), copper (n=4), n=3 unknown. All implants (n=27, 100%) 32 36 IUDs (89%) successfully placed. Unsuccessful placements due pain. One experienced known expulsion. Of 59 successful insertions, 27 (46%) their in place least one year data extraction, 28 (47%) returned follow-up between two weeks insertion. implant required early (i.e. within insertion), whereas (9%) pain and/or bleeding.
